A lifespan perspective on depression in the postpartum period in a racially and socioeconomically diverse sample of young mothers

Background
Consistent evidence from retrospective reports and case registry studies indicates that a history of depression is a major risk factor for depression in the peripartum period. However, longitudinal studies with racially and socioeconomically diverse samples of young mothers are lacking, and little is known about developmental patterns of depression across the lifespan that can inform preventive interventions.
MethodsYoung primiparous mothers (n = 399, 13–25 years, 81% Black) were recruited from a population-based prospective study that began in childhood. Women reported on depression symptoms for at least 3 years prior to their pregnancy, during pregnancy, and at 4 months postpartum. Linear regression models were used to estimate change in pre-pregnancy depression severity and to evaluate associations between patterns of lifetime history and postpartum depression symptoms.
ResultsResults revealed high levels of continuity in depression from pregnancy to postpartum, and across multiple years pre-pregnancy to postpartum. Overall, depression severity leading up to pregnancy decreased over time, but patterns of worsening or improving symptoms were not associated with depression severity in the postpartum period. Instead, area under the pre-pregnancy trajectory curve, representing cumulative lifetime depression burden, was uniquely associated with postpartum depression after adjusting for prenatal depression severity.
ConclusionsDepression in the postpartum period should be considered within a lifespan perspective of risk that accumulates before conception. Clinical screening and early interventions are needed in adolescence and young adulthood to prevent the onset and persistence of depressive symptoms that could have long-term implications for peripartum health.

Pressure-driven laminar and turbulent flow in a horizontal partially filled pipe was investigated using stereoscopic particle imaging velocimetry (S-PIV) in the cross-stream plane. Laminar flow velocity measurements are in excellent agreement with a recent theoretical solution in the literature. For turbulent flow, the flow depth was varied independently of a nominally constant Reynolds number (based on hydraulic diameter, $D_{H}$; bulk velocity, $U_{b}$ and kinematic viscosity $\unicode[STIX]{x1D708}$) of $Re_{H}=U_{b}D_{H}/\unicode[STIX]{x1D708}\approx 30\,000\pm 5\,\%$. When running partially full, the inferred friction factor is no longer a simple function of Reynolds number, but also depends on the Froude number $Fr=U_{b}/\sqrt{gD_{m}}$ where $g$ is gravitational acceleration and $D_{m}$ is hydraulic mean depth. S-PIV measurements in turbulent flow reveal the presence of secondary currents which causes the maximum streamwise velocity to occur below the free surface consistent with results reported in the literature for rectangular cross-section open channel flows. Unlike square duct and rectangular open channel flow the mean secondary motion observed here manifests only as a single pair of vortices mirrored about the vertical bisector and these rollers, which fill the half-width of the pipe, remain at a constant distance from the free surface even with decreasing flow depth for the range of depths tested. Spatial distributions of streamwise Reynolds normal stress and turbulent kinetic energy exhibit preferential arrangement rather than having the same profile around the azimuth of the pipe as in a full pipe flow. Instantaneous fields reveal the signatures of elements of canonical wall-bounded turbulent flows near the pipe wall such as large-scale and very-large-scale motions and associated hairpin packets whilst near the free surface, the signatures of free surface turbulence in the absence of imposed mean shear such as ‘upwellings’, ‘downdrafts’ and ‘whirlpools’ are present. Two-point spatio-temporal correlations of streamwise velocity fluctuation suggest that the large-scale coherent motions present in full pipe flow persist in partially filled pipes but are compressed and distorted by the presence of the free surface and mean secondary motion.

The diagnosis of smear-positive pulmonary tuberculosis in a medical officer working in a metropolitan Australian neonatal intensive care unit led to a contact investigation involving 125 neonates, 165 relatives, and 122 healthcare workers with varying degrees of exposure. There was no evidence of nosocomial tuberculosis transmission from the index case.

Objective
To examine the associations between near and distant visual acuity and biomarkers of Hg, Pb, n-3 fatty acids and Se from the local diet of fish-eating communities of the Tapajós River in the Brazilian Amazon.
DesignVisuo-ocular health and biomarkers of Hg (hair, whole blood, plasma), Pb (whole blood), Se (whole blood and plasma) and n-3 fatty acids (plasma total phospholipids) were assessed in a cross-sectional study.
SettingLower Tapajós River Basin (State of Pará, Brazil), May to July 2006.
SubjectsTwo hundred and forty-three adults (≥15 years) without diagnosed age-related cataracts or ocular pathologies.
ResultsNear visual acuity was negatively associated with hair Hg and positively associated with %DHA, with a highly significant Log Hg × age interaction term. Stratifying for age showed that while young people presented good acuity, for those aged ≥40 years, clinical presbyopia was associated with hair Hg ≥ 15 μg/g (OR = 3·93, 95 % CI 1·25, 14·18) and %DHA (OR = 0·37, 95 % CI 0·11, 1·11). A similar age-related pattern was observed for distant visual acuity in relation to blood Pb, but the evidence was weaker.
ConclusionsThese findings suggest that Hg and Pb may affect visual acuity in older persons, while DHA appears to be protective for near visual acuity loss. In this population, with little access to eye care, diet may have an important influence on visuo-ocular ageing.

This research describes the onset of 13 health conditions and death among 38 adults with spinal cord injuries (SCIs) and 149 adults with traumatic brain injuries (TBIs) compared to adults without disabilities in the same primary care setting, using a retrospective cohort design. The subjects were accrued from an urban and a rural practice, both affiliated with a university family medicine department. Survival analyses, using Cox proportional hazard models, were used to model the onset of each of the health conditions. The results indicated adults with SCI had an increased risk for onset of chronic obstructive pulmonary disease and depression. Individuals with TBIs had an increased risk for onset of anxiety, chronic obstructive pulmonary disease, dementia, depression, and seizures. Neither group had increased risk for asthma, cancer, coronary artery disease, congestive heart failure, diabetes, hypertension, obesity, or transient ischaemic attack. The only conditions with increased risk were those previously identified as secondary to traumatic injury. The other conditions including death did not have higher incidence among the study group, during 7–9 years of retrospective follow-up.

Eggs of the olive ridley marine turtle (Lepidochelys olivacea) have been harvested by generations of Pacific coast communities in Central America for both economic and nutritional reasons. There has been little economic analysis that has identified possible points of improvements for management of the resource. Three egg harvesting projects were studied in Costa Rica and Nicaragua. Field research using semi-structured interviews with stakeholders and key informants were undertaken in June and July 2000. Market price data were collected from harvesters, points of sale and government records. Spread price analysis suggested that more flexible seasonal and regional pricing policies might increase egg profits. Contested property rights have weakened incentives to manage the species as an asset rather than an open access resource. Transaction costs have reduced community efficiency in egg marketing. Community egg marketing cartels with authorized urban selling points are recommended to improve resource management, appropriate a higher egg profit margin and clarify the harvesting origin of eggs for consumers.

A series of conjugated donor-acceptor trienes in which the central double bond is incorporated into an unsaturated isophorone, verbenone or chromone ring has been synthesized. In each case, the donor group consists of an amine and an aromatic or heterocyclic ring system, and the acceptor is the dicyanomethylidene group. The nonlinear optical properties of each of the compounds has been measured and correlated with its structure. The dipole moments and molecular hyperpolarizabilities of these compounds, like those of other conjugated polyenes, are large enough to be used as the active components of electro-optic polymers. Unlike other donor-acceptor polyenes, however, these compounds exhibit the thermal stability required for such applications.

The first volume of the Encyclopéedic announced that Diderot was the author of the work's unsigned articles, and the editor of articles marked with an asterisk. The second volume, however, stated that unsigned articles could be by different authors, including persons who did not wish their identity to be known, while Diderot's asterisk was to prove scarce after the publication of volume VIII, and to vanish altogether after volume x. In the absence of surviving manuscript or other evidence, the correct attribution to their authors of Encyclopédic articles has therefore proved difficult and sometimes a matter for conjecture. Diderot seems to have been responsible for over 5,000 articles or editorial additions in all (many of just two or three lines), of which more than 3,000 appeared in the first two volumes alone. Even when their authorship is not in question, the originality of these contributions often remains doubtful, especially with regard to two subjects on which he wrote extensively: synonyms, for which he drew heavily on the work of the abbé Girard; and the history of philosophy, for which he relied largely on Brucker. Sometimes Diderot cited his source, merely adding a few comments of his own; sometimes he so modified the original text as to make a wholly novel point, not least by diverting his readers' attention to a second article, which informs the real sense of the first.

Diderot drafted his review of Bougainville's Voyage autour du monde in 1771, shortly after the work was published. Although in the Voyage the material on Tahiti occupied just two brief chapters, it was that material which most attracted Diderot's attention; and his treatment of it, together with an amplification of his direct appeal to Bougainville in the review to leave Tahiti as he found it, forms the major part of the first two sections of the Supplément. By October 1772 Diderot had recast and expanded the review as a dialogue within a dialogue which took up themes from two of his short stories that would first be circulated in Grimm's Correspondance littéraire in 1773, and in this new form the Supplément, now described as a ‘Suite des contes de M. Diderot’, was also circulated in successive issues of that manuscript periodical. He thereafter continued to enlarge the work, mainly in the final section, to which some of the additions may even be by his disciple and editor of the 1798 collection of his Œuvres, Jacques-André Naigeon; and he then intercalated the episode devoted to Polly Baker, drawn from the original (1770) edition of Raynal's Histoire des Deux Indes. The Supplément was first published, from an unknown manuscript, and without the Polly Baker episode, by Bourlet de Vauxcelles, in 1796, in a collection of pieces entitled Opuscules philosophiques et littéraires.

Denis Diderot (1713-1784) was one of the most significant figures of the French enlightenment. His political writings cover the period from the first volume of the Encyclopedie (1751), of which he was principal editor, to the third edition of Raynal's Histoire des Deux Indes (1780), one of the most widely read books of the pre-revolutionary period. This volume contains the most important of Diderot's articles for the Encyclopedie, a substantial number of his contributions to the Histoire, the complete texts of his Supplement au Voyage de Bougainville, one of his most visionary works, and his Observations sur le Nakaz, a precise and detailed political work translated here into English for the first time. The editors' introduction sets these works in their context and shows the underlying coherence of Diderot's thought. A chronology of events and a bibliography are included as further aids to the reader.

Catherine's Nakaz was published in Russian in 1767 and in French in 1769. The work was translated into many European languages and was widely read, going through thirty editions in four years. A critical edition by M.D. Chechulin was published in St Petersburg in 1907. An English translation appears in W.F. Reddaway, Documents of Catherine the Great (Cambridge University Press, 1933).
Diderot began writing his Observations sur le Nakaz on his return journey from Russia in 1774. Another unpublished commentary was written by the physiocrat G.F. Le Trosne, under the title L'Esprit de l'Instruction, which has recently been rediscovered in St Petersburg by Georges Dulac. Diderot must have seen this work in 1775, before it was sent to Catherine, because he cites it and uses it as the basis of his discussion of physiocratic ideas in his Observations. From certain parallels between passages in the Observations and the third edition of the Histoire des Deux Indes, which Diderot was working on between 1777 and 1780, it seems that a final revision of the text must have taken place during those years.
After Diderot's death a copy of the work was sent to Catherine, together with other unpublished material and Diderot's library. Catherine reacted angrily to what she read and may have had the copy destroyed; no version of the work has been found in St Petersburg.
